			<description><![CDATA[ Our nursing staff says that we are not to do a breathing treatment before they attempt to gain I.V. access. They say that something in the albuterol causes the veins to blow after a treatment is given. I think it is just them trying to get the patient before we get them. Does anyone have any studies to prove that albuterol causes veins to blow during I.V. access? I need something to prove this is false. We have had him to the Allergist and the Asthma Specialist.  Nothing seems to help. He has done all the inhalers and pills. As the season goes on the attacks become daily at practice or a meet. In my Respiratory Care Pharmacology book by Rau, it states that the lung deposition of MDI's w/ &amp; w/o spacers, DPI &amp; SVN ranged from 8.8% for MDI, 9.1% for DPI, 12.4% for SVN &amp; 13% for MDI with spacer. My question is does anybody know the percent amount of lung deposition for an ultrasonic nebulizer. With a smaller particle size than a conventional SVN, I would think that it would be... ]]></description>

Is there any advantage that anybody knows of using this over racemic epinephrine? I didn't ask anybody, my guess would be that it's a cost issue. 			<title><![CDATA[ Tobramycin down the ETT ]]></title>

			<description><![CDATA[ I was wondering if anyone else is using Tobramycin to treat gram-neg bacterial infections in patients with  a tracheostomy tube. We have one Doc in the ICU who has requested that the RT's instill 40 mg (5cc total) directly down the trach or ETT twice a day to fight Gram-neg bacterial pneumonia.  The patient's are still receiving their IV antibiotics but there is one study that he sights that shows that direct instillation will lower and eventually eradicate the bacteria in the lungs thus... ]]></description>

We have a couple of times with great sucess but I have a child in my asthma program on flovent with 3-4 weekly asthma flare ups. 			<description><![CDATA[ Is anyone out there using nebulized decadron? We did so in Idaho with wonderful results in asthmatics, croup and severe COPD exac. We usually used 4mg/ml vials and used anywhere from 2mg to 10mg doses.I know it is not approved for this route by the FDA (kind of like Terbutaline) but am curious if anyone is using and do you have a written policy?
